If you and your family chose us at Robertson Chiropractic, there are certain things that you can do to help speed up the paperwork process so we can better serve you.
- First, congratulations! You’ve already taken your first step towards making a change towards improving your health. Please download and complete the new patient forms found on the link below and bring it to your first appointment. If you are unable to complete it before your first visit, then we have forms you can fill out in the office.
- Second, contact our office so we can schedule an appointment time which works best with your schedule. Our contact information is located on the right side of this page.
- Third, is your first appointment. A one-on-one thorough musculoskeletal evaluation will be performed. Various things will be evaluated, including other health factors such as diet, exercise and sleep habits. Together, you and the doctor will discuss what your goals are for your treatment, so that your treatment plan can be individualized for you.
- Fourth, Dr. Robertson will review your file, and determine if you are a good candidate for Chiropractic care.
- Fifth, once your file is reviewed, you will schedule your next visit with Dr. Robertson to receive your new patient packet and to discuss the results from your examination, financials and any other questions you may have.
